Academic Conduct Code
Boston University’s Academic Conduct Code is designed to assist in the development of a supportive and productive learning environment. It is both a description of the University’s ethical expectations of students as well as a guarantee of students’ rights and responsibilities as members of a learning community. The Code provides clarity related to policy and procedure regarding academic conduct.
Access to Educational Records Request
To inspect education records, a student must submit an Access Form identifying the records they wish to review. The form should be submitted to J.P. Matychak (firstname.lastname@example.org) Associate Dean of Student Experience at the Questrom School of Business. This form is also available in Office of the University Registrar.
Requests to access records will receive a response within forty-five days of submission. A University official will arrange for access and will notify the student of the time and place where the records may be inspected. The University may charge a fee for copies of requested education records.
Business Card Orders
To order your BU business cards online, create a username and password on the login page. Please enter your local mailing address, as cards will ship directly to you.
Comps, Proposal, Dissertation Petition Form
Doctoral students must submit this from to take the Comprehensive Exams, or to defend their Proposal or Dissertation, at least three (3) weeks in advance of the date of the exam or proposal. The Doctoral Student and Faculty Advisor must sign this form, and then brought to the Graduate Academic & Career Development Center for the final signature by their Academic Advisor.
Concentration Declaration Form
All concentrating students are required to complete this form.
Conference & Research Funding Proposal
PhD in Management and PhD in Mathematical Finance Student may use this form to apply for Conference & Research Funding. Deadlines to submit this form are September 1st, December 1st, May 1st and June 1st.
Course Adjustment Form
The Graduate Academic & Career Development Center must receive this form within four business days of the date of the instructor’s signature.
Course Overload Policy & Tuition Waiver
Standard full-time tuition covers 12-18 credits per semester. All full-time students can automatically take up to 18 credits per semester. Overloading is registering for more than 18 credits in a semester. There are different overload policies varying by program and start date. Program policies can be viewed on the form.
Cross Enrollment Form
In order to apply for registration for graduate courses at the Questrom School of Business, you must be an enrolled, degree-seeking graduate student in good standing within Boston University.
Curricular Practical Training (CPT) Form
Curricular Practical Training (CPT)* is employment authorization required of international students within F-1 immigration status to legally participate in paid internships in the United States. Graduate students at the Questrom School of Business must follow not only the procedures and regulations as outlined by the United States Citizenship and Immigration Services (USCIS) and the Boston University International Students & Scholars Office (ISSO), but also those outlined by the Questrom Graduate Academic & Career Development Center.

Directed Study MBA/PEMBA
If you wish to conduct research in a specific area of interest under the guidance of a faculty member, you may apply for a directed study. A directed study may not substitute for a required course or duplicate an elective course already completed or that is offered at the college. Upon completion of the directed study, you will receive a grade from the sponsoring faculty member. The grade will be included in the computation of your grade point average.
Directed Study MSMS
If you wish to conduct research in a specific area of interest under the guidance of a faculty member, you may apply for a directed study. Upon completion of the directed study, the student will receive a grade from the sponsoring faculty member. The grade will be included in the computation of the student’s grade point average.
Doctoral Program Yearly Report
Doctoral Students are required to complete a yearly report at the end of each academic year. Students will recieve notification from their academic advisor when the report is available.
Doctoral Dissertation Announcement
Your academic advisor in the Graduate Academic & Career Development Center is required to email the Questrom community this two weeks prior to your defense date. As such, they will need the text of your defense announcement more than two weeks before the scheduled defense date.

Global Experience Course Applications
Graduate Elective Request
With prior approval, students may take management-related electives at other graduate schools and colleges within Boston University. Please complete this form, and return it to the Graduate Academic & Career Development Center.
All potential graduates are required to submit this form to the Graduate Academic & Career Development Center. For students planning on graduating in May or September, the application for graduation is due November 15th. For students planning on graduating in January, it is due April 15th. Once you have been confirmed as a tentative graduate you will receive an email from your advisor with further details on your graduation status, as well as important information such as updating your diploma name and diploma pick up information on the Student Link.
Intercultural Communication Program Enrollment
Students must enroll to participate in this program by September 9th, 2016.

Program Change Form
Public & Nonprofit Management Program Internship Waiver
PNP students are required to complete a 300-hour paid internship in a public, private, or nonprofit organization. Students selecting a private sector enterprise must focus their work on corporate social responsibility, sustainability, cause marketing, social impact, philanthropy, or work for a socially responsible company. PEMBA students may waive the internship requirement with applicable work experience, pending permission of the PNP faculty director.
Registration Petition for the Boston Area Consortia
Registration in the Boston Area Consortia is a formal agreement between Boston University and several local colleges and universities whereby BU students can attend courses at other institutions, and students from other institutions can attend courses at BU. The primary schools in the Cross Registration consortium are Boston College, Brandeis University, Tufts University, and Hebrew College.
Your advisor or dean, the BU Registrar’s Office, and the host institution’s registrar’s office must sign the completed Cross Registration Petition. The copy of the signed Cross Registration Petition must be submitted to both the BU Registrar’s Office and the host institution’s registrar’s office for official registration.
PhD students must complete this form within 20 days of travel in order to be reimbursed for conference travel. The deadline for research reimbursements may be extended.
